_____Scanned copy of Diploma and/or Credential to practice as a veterinary technician.

_____Signed copy of Waiver, Release and Indemnity Agreement Form.

_____Electronically completed Professional History and Employment History Form.

_____Electronically completed CE form with scanned Proof of Attendance and outline of continuing education courses attended specific to the applicant’s subspecialty.

_____Scanned copy of completed Advanced Veterinary Internal Medicine Technician Specialty Skills Form in area of specialization.

_____Scanned copy of completed Advanced Veterinary Internal Medicine Technician Specialty Knowledge List Sign off sheet

_____Electronically completed Case Log Form.

_____Four In-Depth Case Reports submitted as word documents.

_____Two letters of recommendation. (Sent separately by the letter writers)

_____Three electronically completed Potential Examination Questions specific to the area of specialty.

_____One complete application packet, in a folder including one CD-R or thumb drive with the complete application except as noted paper documents. The CD-R should be labeled with your name.

_____Application fee. Check in the amount of $50 dollars made payable to AIMVT.
