Advanced Skills and Procedures I Performed:  aseptic placement of IV catheter for administration of anesthesia induction agent, Propofol 10mg/ml concentration (4mg/kg IV, total dose administered 1.6 cc IV) followed by intubation with endotracheal tube. Maintained anesthesia with Isoflurane (1.5%) and O2 1L/min for rhinosocopy and radiation therapy; anesthetic monitoring with pulse oximetry and endtidal CO2; phlebotomy for "SNAP" FIV/FELV tests; administration of 800cGy megavoltage radiation bilaterally from cranial nasal planum to caudal extent of orbits and encompassing full width of described area using plan constructed by board-certified radiation oncologist; client education on radiation therapy side effects.

 

Advanced Skills and Procedures I Assisted With: set up equipment for rhinoscopy and biopsy procedure; transferred patient to ICU for recovery and took dictated medical orders for overnight hospitalization; oversaw handling of biopsy samples for proper packaging and delivery to outside histopathologist

 

Outcome: after initial diagnosis and only one radiation treatment, the owner discontinued therapy due to financial constraints. Pet is currently seeing RDVM for follow-up and is receiving only prednisone from RDVM at 2mg/kg/day PO (9kg compounded as a syrup) as therapy for his lymphoma. Pet is reported to be stable, with moderate appetite.

Fentanyl CRI: 3-5mcg/kg/hr X 36hrs (Skill 12, 13, 14). Monitoring: HR, ECG, SPO2, ETCO2, RR, NIBP, & Temp. Post-op pt anxious & restless, decreased fentanyl CRI to 3mcg/kg/hr: pt relaxed and sleeping (Skill 31). Anticonvulsant therapy: levetiracetam (12.5mg/kg) 100mg PO TID, clonazepam (0.01 mg/kg) 0.125mg PO BID & diazepam CRI (0.3mg/kg/hr) calculated & ready if needed for seizures (Skill 9, 10, 11).

Advanced Skills & Procedures I assisted with: Sample collection & submission for histopathology (Skill 16). Proper pt handling: quietly & gently, minimal restraint, nothing around neck and avoid raising pt’s blood pressure (Skill 23).

Outcome: Great recovery: patient seizure free with normal mentation at 3 & 6 month rechecks.
